The costs and requirements to upgrade Covenant gear are:
5 and some Valor that you want to spend, you’ll need to head to the Valor Point upgrade vendor in Oribos. The Valor Point upgrade vendor in Oribos is the same as the PvP one – Aggressor Zo’dash. This NPC can be found at 34.55, 56.55 in Oribos and now functions as both the PvP and Mythic+ upgrade vendor.
There is a maximum cap on how much Valor can be earned. In the first week this will be 5,000 Valor. This will increase by 750 each week. In the second week of the patch it is 5,750, third week it is 6,500 Valor, and so on and so forth.

All the higher-rank upgrades (from Rank 6 to 7 and above) are gated behind the new Mythic Scoring System in Patch 9.1. here
Mythic +0 Dungeon Gear that starts at Item Level 210 (Rank 1) can be upgraded to Rank 6 (Item Level 226) without any requirements. here
This also applies to the Great Vault loot. For example, if you receive a piece of gear that has Item Level 226 from the weekly chest, it will be of Rank 6 out of 12.
Mythic+ Dungeon Gear Ranks
Starting in Patch 9.0.5, new gear from Mythic+ Dungeons will receive a rank from 1 to 12 based on the item level. Upgrading an item with Valor will only increase it by one rank.
